Mathematicians are not supposed to have a great sense of humor like you do. I think several thousand operations are great. You should do fine with that surgeon team. I don't know what he means when he says you have a low score. Did he elaborate? Let me know please and keep on cracking the jokes. Say, Allan, I just noticed... You have a great big huge yellow head. I am so glad that you got rid of the first guy, but how many operations has the second guy performed. This is very important because it takes about 500 to 750 operations to do the Robo Lap Oper skillfully. It's OK to ask the Doc. this. Basically you look for a Doc. who has done a lot of ops with HIS best proceedure.George

What type of PCa surgery are you having? Open, Lap, Robotic? Takes two to write.! Now I don't chase butterflies I chase fossiles like you and me. Actually they were micro formaniferia found in the ancient sea around chicago. My best lawyer buddy is from chicago. Going back there with him this summer. One of the new guys that joined worked in Chicago as a Mechanical Engineer for 20 years. Keep up you great spirit.. George. .

I dunno why anyone would think that the side effects of radiation are any better or worse than the side effects of surgery. It all depends on how good the surgeon or the radiotherapist is! Ask any man who has radiation cystitis and/or radiation proctitis that starts 3 years after radiotherapy for prostate cancer!

Re George ... Don't worry. George and I have a very strong relationship based on other issues. He will survive my comments -- and then take a dig at me about something else whenever he gets the chance!

Dear Allan:

You appear to have what most urologists would define as low risk prostate cancer. (PSA < 10 ng/ml; Gleason 3 + 3 = 6; 1/12 cores positive for cancer). Thus, when your urologist says that this is no reason to have surgery, he does have a good point, because there is a very good chance that your cancer is "latent" (in other words, it will never actually cause you a clinical problem).

Having said that, you appear to also have other prostate problems that sound like symptoms of prostatitis. Obviously I am NOT a urologist and I cannot diagnose your condition, but I can tell you that when I have heard symptoms like this described before, prostatitis has commonly been shown to be an underlying issue.

So what to do? Plenty of men of your age with a diagnosis of cancer like yours do have surgery. And plenty of urologic oncologists will operate on men with your diagnosis. So if you go and get a second opinion, you can very likely find a surgeon who will listen to your plea. It is also very possible that a radical prostatectomy will resolve your other problems.

However (and it's a big however), if you decide you want to go looking for a surgeon who will operate on you, you want to make quite certain that it is someone with an extensive track record. Given the fact that you are otherwise in good health, you presumably have every desire to end up fully continent and fully potent post-surgery. This can never be guaranteed, but the experience and skill of the surgeon can profoundly impact the outcome.

Dr. K''s prior comments may (at least to some extent) reflect the fact that you are not his patient and that he doesn't want to be seen to be "second guessing" one of his peers. However, in the end this decision is down to you.

The whole trick to managing any illness is to understand competing risk. So if a man is at very high risk of cardiac death, as might be suggested by severe obesity, then he is at lower relative risk of prostate cancer death than a man not at high risk for cardiac death. So yes, a very very obese man needs to think very carefully before he chases down his PSA, biopsy, and surgery.

I am not trying to suggest that we have science behind all this and that we have absolutely precise predictive models of time-to-death or cause-of-death. In fact, I know we do not because I tried very hard to develop such models. All the validations of human and computer modeling showed were not much better than flipping quarters on specifics. Anyway, back to your situation, surgical time is one measure of surgical quality. It is worth noting.
