Yes I have BPH, but have read when you take Finasteride your PSA is lower.

Dear John;

First, your lack of faith in PSA testing is understandable. However, what doctors all too often fail to tell patients is that PSA testing and responses to it are individualized. Some men just have normal PSA levels that are lower than others, and some have PSA levels that are higher. The numbers people use as guidelines are just averages. You appear to be a part of the "lower than average" group.

Second, if you decide to enter the abiraterone actetate trial I do NOT believe that you will necessarily progress any faster on the placebo arm of the trial. Stopping the Casodex is something that would be the normal next step in your current treatment anyway because of an effect called antiandrogen withdrawal. In many patients on drugs like Casodex, stopping that drug will -- for a short period of time (of around 6 months) -- actually lead to a decline in the PSA level. My understanding is that you will only actually be allowed into the abiraterone trial AFTER you have stopped the Casodex AND your PSA either continues to rise or first drops and THEN starts to rise again.

And you should know that there are lots of other potential new drugs coming into trials after the abiraterone, so keep your chin up. We are WAY ahead of where we were a decade ago!
