After husbands diagnosis on 5/29/09 began to do research on PC. Found Dr Mani Menon, highly recommended. Husband had radical prosectomy on 7/14. Pathology report complicated. I have many questions

Claire:

There are two types of PSA test -- the standard one (which will measure PSA levels down to 0.1 ng/ml and report anything lower as either <0.1 ng/ml or "undetectable") and the ultrasensitive PSA test (which will measure PSA levels down to 0.03 ng/ml and report anything lower as either <0.03 ng/ml or "undetectable").

To all intents and purposes, any stable PSA level lower than 0.1 ng/ml after a radical prostatectomy is what you are looking for. I really don't think that whether your husband is taking Propecia or not will affect this result by comparison with the surgery.

After surgery your hubbie's PSA should go to < 0.1 ng/ml, and that's where it will stay for good if Dr. Menon was able to get all of the cancer out. Using Propecia will not affect than one way or another. I am assuming that you have a post-surgical PSA test scheduled for some time in September or early October to find out what it is after the surgery. Yes?

However, if Dr. Menon wasn't able to remove all of the cancer, and hubbie's PSA either hasn't gone down to < 0.1 ng/ml, or starts to rise again, Propecia won't be strong enough to stop that happening.

Oh, and I am glad to hear that that Gleason score was only 7 in the pathology report. That's good. It is actually relatively uncommon for the Gleason score to be lower in the prostatectomy pathology report than it was in the biopsy report.

I am not aware of any detailed info on the post-prostatectomy use of Propecia, but I can't imaging it would do any harm since it is only a low dose of finasteride (one fifth of the standard daily dose used in the Prostate Cancer Prevention Trial).

Just make absolutely sure that your husband's doc knows that he is taking it.

Hi Claire.

The Gleason 8 is not so good. Was it still Gleason 8 in the pathology report or had it gone up to Gleason 9? Also, I suspect that the clinical stage of T2b may have turned into a pathological stage of T3a or T3b. On the other hand, at least the PSA was still on the low side, which is promising. Let me know if you want to talk through the pathology report together.

Just fire away with the questions OR ... if you prefer, go to my page, click on the "Send a Message" link (under my photograph) and leave me a private message with a phone number and a couple of times when I can call you. We will do our very best to answer all of your questions for you! Here is some other information we are going to need to know (in addition to whatever is in the pathology report):

-- What was your husband's PSA at the time of diagnosis?
-- Does he have any other health issues (diabetes, cardiovascular problems, etc.)?
-- What was his original clinical stage (prior to treatment)? T1c?
-- What was his original biopsy-based Gleason score (e.g., 3 + 3 = 6)?
-- How many positive biopsy cores did he have (out of the total number taken)?
-- How old is he?
